I actually could do it when I was in my teens and early 20s -- However, I found it was something like tickling yourself, and not that erotic Of course the whole position being so uncomfortable might have something to do with that!

What worked best was to jerk off almost to orgasm and then go down. At that point the cum-reflex took over on the dick side and I could enjoy it from the mouth side without having to "tickle myself" over the top.

I really wish I was still flexible enough to do this.
Now I can lick the head and occasionally get a bit of it to my lips, but once upon a time I could get to the base, and could go from start to finish hands free.
Sadly I got old (relatively speaking) and out of shape, so no more of that.
